Guidance on informed consent

Informed consent can be defined as: permission granted in the knowledge of the possible consequences, including risks and benefits.
Components of an informed consent document:
- About you, your organization
- Purpose of the project (the reason why you are collecting this information)
- Data security and confidentiality -- how do you plan to protect the information?
- How the person can provide their consent (verbally, written, other)
- Benefits of participation
- Risks of participation (examples: psychological discomfort, risk of sharing sensitive information), and are there ways to mitigate these risks?
- Their rights as a participant
- Participation costs
- Compensation
- How you plan to use the information you have received (examples: public dissemination, collaboration with accountability mechanisms)
- Contact information if they have questions or concerns
